Except as provided for in paragraph (d) of this section, a covered veteran eligible for the Veterans Community Care Program under §17.4010 may choose to schedule an appointment with a VA health care provider, or have VA authorize the veteran to receive an episode of care for hospital care, medical services, or extended care services from an eligible entity or provider when VA determines such care or services are clinically necessary. (b) Selecting an eligible entity or provider. A covered veteran may specify a particular eligible entity or provider. If a covered veteran does not specify a particular eligible entity or provider, VA will refer the veteran to a specific eligible entity or provider.

The provider I am requesting is Accommodating Home Care, a credentialed provider for veteran in-home supportive services through the VA Community Care Network (VA CCN). I have made several attempts, at many different levels within the VA system, to have my provider choice honored, as outlined
